Respiratory specialist nursing and home oxygen review

The respiratory specialist nursing and home oxygen review service offers care to patients diagnosed with respiratory conditions, either in their homes or nearby settings.

Respiratory specialist nursing

The team see patients who have a confirmed diagnosis of significant respiratory disease (but not Asthma) who are:

  • over 18 years old and registered with a Doncaster GP
  • struggling to manage their condition due to worsening symptoms
  • are attending A&E
  • at risk of hospitalization due to the complexity of their condition

A holistic assessment is done with the patient and/or carers and a written management plan developed.
Referrals may be made to other relevant services as determined on assessment.

Once we have established a plan with you or your carers you will be discharged back to the care of your GP.

Following discharge, the service is accessible for advice. Please be aware that we do not recommend or supply nebulisers.

The home oxygen review

The service does not initiate oxygen but reviews all adult oxygen users who have a Doncaster GP.

The aim of the service is to ensure safe effective and appropriate use of prescribed oxygen therapy. Patients are all seen on a six-monthly basis or when there is a clinical need identified in line with National Guidance.

The capillary blood gas service

Carry out capillary blood gases in the patient’s own home at the request of a respiratory clinician, to ensure that patient is receiving the appropriate amount of oxygen.

How to get help

All referrals to be made via the single point of access by either GP or another clinician. Once received the team will triage the referral and the outcome will be communicated with the referrer.

Patients who have previously been under the care of the service can self-refer and will triaged in the same way and if appropriate re-registered.

The service is open 9am to 5pm, Mondays to Fridays (No weekends or bank holidays).
