Primary care mental health is a service in Rotherham, Doncaster and North Lincolnshire
The Primary Care Mental Health team delivers mental health support to adults, supporting GPs in addressing mental health concerns within primary care.
How we can help
The service offers support to patients over 18 years of age, with moderate to severe mental health needs.
Our focus is on fostering a person-centred approach to both physical and mental health, encompassing consideration of your social care needs.
Assessments of mental health are conducted either face-to-face or by phone. Outcomes may involve:
- guiding and referring you to other services or organisations
- providing treatment advice
- conducting physical health checks
- offering brief interventions to help manage mental health challenges
Doncaster
The Doncaster primary care mental health hub provides triage, holistic assessments and a variety of interventions to aid your recovery. We may also refer you to partner agencies if your needs can be better met.
Rotherham
The Rotherham primary care integrated mental health hubs, the hubs are aligned to the primary care networks across Rotherham. We offer mental health assessments and advice.
North Lincolnshire
The North Lincolnshire Primary Care Network team operates through three pathways:
- the routine pathway, offers routine and non-urgent specialist mental health assessments, advice, and referrals
- the dementia review pathway, monitors and supports individuals on prescribed dementia medication, conducting yearly medication review appointments post-diagnosis from secondary care
- the severe mental illness (SMI) pathway, provides an annual health and medication review for those diagnosed with severe mental illness, collaborating with GPs to monitor physical health
How to get help
Doncaster
The Doncaster service does not accept self referrals. We accept routine referrals from your GP or health professionals.
Rotherham
Referral is primarily done via the primary care mental health practitioners working within Rotherham GP surgeries. Other mental health support services can help with referrals.
North Lincolnshire
Referrals to our service are typically made by professionals like your GP. Additionally, you can self-refer through the single point access (SPA).
