Adult community mental health
Adult community mental health is a service in Rotherham and Doncaster
The team provides assessment, intervention, and treatments for people experiencing complex mental health difficulties which significantly impact their daily life. This may include complex emotional needs, anxiety and mood disorders, complex trauma, psychosis, and other severe and enduring mental health conditions.
How we can help
- Review and formulation, we review individual mental health conditions and assess the most appropriate pathway within the treatment team.
- Treatment planning, we develop time-limited personalised care plans that may include therapy, short term interventions, and longer-term treatment, medication management, and other support services from different disciplines.
- Therapeutic interventions, we offer individual and group therapies, such as cognitive-behavioural therapy (CBT), psychology, and structured clinical management (SCM).
- Duty service, we respond to mental health concerns between the hours of 9am and 5pm for existing secondary care patients already open to the service.
- Case management, we coordinate care via a key worker and signposting individuals to resources such as housing, employment support, financial assistance, and third sector support.
- Education and prevention, we help raise awareness about mental health, reducing stigma, and promoting preventive measures.
- Support services, we provide ongoing support to help individuals with their families and carers manage their conditions and symptoms and improve their quality of life.
These teams often include psychiatrists, psychologists, social workers, nurses, occupational therapists, and peer support specialists who work collaboratively to address the mental health needs of the community.
How to get help
To address ongoing mental health issues, visit your GP to discuss your concerns. Your GP can initiate a triage process with a specialist mental health practitioner in primary care to determine the most appropriate pathway for your care. This may include:
- initial assessment, your GP will conduct an initial evaluation of your mental health status
- triage, if needed, your GP will refer you to a specialist mental health practitioner in primary care for a more detailed assessment
- care pathway, based on the assessment, the practitioner will help establish the best care pathway, which could involve therapy, medication, or other interventions
- referral to secondary services, if your condition requires more specialised care, the practitioner will refer you to secondary mental health services for further treatment and support
This process ensures you receive the most appropriate and effective care for your mental health needs.
