Older people’s community mental health
The Community Mental Health team (CMHT) offers assistance to older people facing mental health issues. The team provides assessment, treatment and care management for patients over the age of 65 in Doncaster, Rotherham, and North Lincolnshire.

Our service supports older adults with mental health concerns, both at home and in clinic settings, enabling them to manage challenges and remain at home.
We provide assessments, treatment interventions, and support for patients with mental health issues, as well as their caregivers.
How to get help
Doncaster
Referrals to the Community Mental Health team service are mainly made by the individual’s GP or healthcare professional. Self-referrals are only accepted in crisis situations.
Rotherham
To access this service, referrals can be made by contacting the Rotherham care co-ordination centre at 01709 426600.
North Lincolnshire
Referrals to this service are received through local general practitioners (GPs).
