Mindset

Mindset is a service in Doncaster. It is a dementia outreach programme.

Our mission is to change the mindset surrounding dementia diagnosis. We hope to widen access to dementia support at all stages of people’s journey and deliver wraparound care.

How we can help

Mindset is a wraparound care service that assists an individual through their entire journey.

Tier 1

This is for those who are pre-diagnosed but have experienced mild cognitive impairment and need signposting for support. It is handled by our dementia advisors and memory advisors.

Tier 2

Tier 2 specialises in cases where a dementia diagnosis has been given and patients or family and friends are seeking support. Our specialists (Dementia advisors) will offer advice and education to promote wellbeing and social inclusion.

Tier 3

Tier 3 is dedicated to those patients that require clinical advice intervention. Our clinical dementia advisors which comprise registered nurses, allied health professionals, and occupational therapists.
