Community stroke rehabilitation is a service in Doncaster

We deliver high-quality, personalised, interdisciplinary rehabilitation for stroke survivors and their families within community settings.

How we can help

We are a rehabilitation team that provides rehabilitation and guidance to people who have suffered from a stroke within the last 6 months. Our team has the following staff:

  • specialist nurse
  • physiotherapists
  • occupational therapists
  • speech and language therapists
  • clinical psychologist
  • assistant practitioner
  • rehabilitation assistants

We conduct individual assessments and provide therapy tailored to the patient’s personal goals as agreed upon.

We offer rehabilitation in our clinic at Evergreen, Tickhill Road Hospital. For those who are housebound, we can offer home visits. By offering clinic appointments, it allows our staff to devote more time to patient care and less time to travel. It also allows you to meet others affected by stroke.

Your first appointment will usually be a one-to-one assessment. We will then offer you follow up appointments as a mixture of one-to-one and group sessions, depending on your needs.

How to get help

Referrals typically come from the stroke units at Doncaster Royal Infirmary ward 16 or the Mexborough Montagu rehab ward.

Additionally, referrals may be made by GPs, social services, or other healthcare professionals.
