Care home liaison is a service in Rotherham, Doncaster and North Lincolnshire
The Care Home Liaison team (CHLT) offers assessments and interventions to support patients dealing with various mental health issues, including dementia, residing in care homes.
This service is accessible to patients over 65 living in residential care or nursing homes, and those under 65 diagnosed with dementia.

We engage with residents, families, and care homes to establish and maintain evidence-based care plans that meet their specific needs.
How to get help
Doncaster:
Referrals are received through the older people’s mental health liaison single point of access (SPA). Upon receiving the referral, a senior clinical nurse reviews all referrals on the same day and communicates the outcome to the referrer.
Rotherham:
Referrals can be made through the single point of access (SPA)
North Lincolnshire:
Referrals to this service can be initiated by your doctor requesting a visit from the service to your care home.
