Self-referrals are not accepted, patients are referred through the Patient Flow team.
Rotherham the Woodlands
Not a self-referral service, referrals are usually received from other teams or healthcare professionals.
North Lincolnshire Great Oaks mental health unit
Referrals accepted from healthcare professionals like GPs. In crisis situations, patients can make self-referrals by calling 08000 804899.
